The purpose of this course is to provide learners with the knowledge, skills, and confidence to support participants who live with multiple chronic conditions. These conditions, such as diabetes, arthritis, cardiovascular disease, dementia, and respiratory illness, often interact with each other in complex ways. This makes care planning more challenging but also more rewarding when done effectively. The course emphasises safe, ethical, and person-centred practices that comply with Australian standards and the NDIS Practice Standards. By the end of the training, learners will understand how to identify risks, implement practical strategies, and provide holistic support that respects dignity, independence, and participant choice.
