Course Description

The purpose of this course is to provide learners with the knowledge, skills, and confidence to support participants who live with multiple chronic conditions. These conditions, such as diabetes, arthritis, cardiovascular disease, dementia, and respiratory illness, often interact with each other in complex ways. This makes care planning more challenging but also more rewarding when done effectively. The course emphasises safe, ethical, and person-centred practices that comply with Australian standards and the NDIS Practice Standards. By the end of the training, learners will understand how to identify risks, implement practical strategies, and provide holistic support that respects dignity, independence, and participant choice.

Course Outcomes

By the conclusion of this course, learners will be able to:

  1. Define and explain multiple chronic conditions: Understand what multimorbidity means, how it is diagnosed, and its impact on participants.
  2. Identify risks and hazards linked to complex care needs: Recognise safety issues such as falls, medication errors, malnutrition, and social isolation.
  3. Apply safe, person-centred care strategies: Use tailored approaches that respect dignity, independence, and individual goals.
  4. Communicate effectively with participants, families, and health professionals: Develop skills to share information clearly, support decision-making, and collaborate within care teams.
  5. Align practice with NDIS Practice Standards and Australian health guidelines: Ensure all care delivery is compliant, ethical, and focused on achieving quality outcomes for participants.
